Below are the documents you need to register for our Public Assistance 5-week CNA Program in partnership with WorkSource Georgia. Click on the link  https://middlegaworks.com/ , then click "apply now". 

Please upload your:
1. Copy of your Driver’s License/Picture ID
2. Copy of your Social Security Card
3. Copy of your Birth Certificate and (Children’s if under age 13)
4. Proof of income (Last 6 months need start and end dates of jobs)
5. Proof of residence (Utility Bill, Mail, etc.)
6. Copy of DD-214 (for prior military)
7. Public Assistance Verification (If Applicable)

8. Family Composition and Address Verification (Must be signed by a 3rd Party who does not live in the household)
9. Customer Contacts ( Fill in all blanks for all five contacts)
10. Separation Letter (If Applicable)
11. UI Benefits Award Letter (If Applicable)

- Out-of School Youth desiring to take the course must be between the ages of 16-24 and have a barrier and be low-income; Other individuals not in this age range must receive one of the criteria listed above. 


- students must pass the literacy test (reading, math, language) administered by Work Source Georgia.


- TB (PPD) skin test dated within 1 year of applying to the CNA program (the cost of this isn't paid by WIOA); or $30 TB/PPD skin test administered & read by Vision NATP Nurse (if student doesn't have one dated within a year)

- Copy of COVID-19 vaccination record (if vaccinated) 


- student MUST be able to attend class on Tuesday, Wednesdays & Thursdays according to the CNA course they choose to enroll in (​read the description for each). Please note, you can only miss one (1) day of class. 


- must reside in one of these Georgia state counties: Baldwin, Bibb, Crawford, Houston, Jones, Monroe, Peach, Pulaski, Putnam, Twiggs or Wilkinson.  


- once your application is approved by WIOA, you will receive an emailed letter of acceptance from our Program Coordinator. 


- after successfully completing the course, the student will receive a certificate. The student then qualifies to take the Credentia CNA365 exam. Please note, the student will be responsible for the current cost of the Credentia CNA365 state exam. 

- any student having physical (ex: back problems, pregnancy, surgeries, etc.) restrictions, must obtain clearance from their licensed Medical Doctor, Nurse Practitioner or Physician Assistant. please choose a physical assessment form below. 


Vision Nurse Aide Training Program, Inc. has now partnered with Georgia WIOA (Workforce Innovation and Opportunity Act) Work Source program!! This program was started in August 2023 to provide low-income individuals an opportunity to​ train in our 5-week CNA course to learn the basic nursing skills to become a CNA (Certified Nursing Assistant).
